How exactly does Mob Mayhem work? Most importantly, do you need to attack the model before you can declare the tactic on it or can you declare it on a model you haven't attacked yet and it's ability takes place when you attack it?

You use the tactic before you attack. Every model of that unit that attacks an enemy model that already has been hit during the same attack action gets the bonus automaticly.
For example you have a unit of 6 tribesmen who are engaged with a cerberus and 2 hoplites. 4 of them can attack the cerberus while the other two each have only one hoplite within their melee reach. Before the attack, they perform the Mob Mayhem tactic. The first one makes a normal attack against the cerberus and misses (they can choose a fighting style as normal, lets say they take the +1 MEL cause of the high DEF of the cerberus and the hoplites). The second one also attacks the cerberus, scores a hit and makes a normal damage roll. The other two tribesmen who also can attack the cerberus now get the +1 MEL/+1POW bonus from Mob Mayhem since the cerberus has been hit before during the same attack action. The two tribesmen who attack the hoplites must make their attacks without any bonus from Mob Mayhem.
If it comes to a second round of attacks, the tribesmen have to hit the cerberus again, before the remaining guys get the Mob Mayhem bonus.
